The main engine is the main driving engine on a ship which plays a very important role in the smooth running of the ship. This research aims to determine the influence and determine what efforts should be made to overcome the cooling system leak in the left main engine cylinder head on the KM Mutiara Ferindo V ship. The author summarizes all the problems and tries to analyze the problem using the analytical method, namely fishbone analysis method. Based on the analysis, it was found that the problem of fresh water cooling system leaks in the cylinder head was caused by too hot of the main engine which could cause the rubber o-ring on the seating valve to melt and that was what caused the water in the fresh water cooler to enter the cylinder liner which would cause the gas temperature to Exhaust will be low if left untreated, spare parts used have exceeded working hours and the pressure on the fresh water cooling pump will drop. This fresh water cooling system leak can affect the performance of the main engine. In conclusion, with this problem, it is important to care and maintain both the fresh water cooling pump and the main engine and pay more attention to the engine if it has entered the replacement of components that have had high working hours
